By enrolling my child in Camp60 youth programs, I acknowledge and agree to the following:
1. Assumption of Risk
I understand that physical activity involves inherent risks, including but not limited to: falls, collisions, sprains, strains, bumps, bruises, and other potential injuries.
I voluntarily allow my child to participate and accept all risks associated with participation.
2. Release of Liability
I agree not to hold Camp60, its coaches, staff, owners, volunteers, or partners liable for any injury, accident, illness, or damages that may occur during participation, on or off premises.
This release applies to all youth programs, including but not limited to:

After-Care Program
-
Homeschool PE
-
Outdoor activities
-
Fitness activities
-
Games and conditioning
I understand this release is valid for the entire duration of my child’s enrollment.
3. Medical Treatment Authorization
In the event of an injury or emergency, I authorize Camp60 staff to administer basic first aid and to contact emergency medical services if necessary.
I understand that I am responsible for all resulting medical expenses.
4. Behavior & Safety Expectations
I agree to support Camp60’s safety rules, including:
Camp60 reserves the right to dismiss a child for unsafe or disruptive behavior.
5. Photo/Video Permission (Optional)
(Parents can initial one)
YES, I allow Camp60 to use photos or videos of my child for social media and marketing.
NO, I do not allow photos or videos of my child to be used.
6. Allergies & Medical Conditions
I confirm that I have disclosed all allergies, medical conditions, or special needs that may affect my child’s participation.
7. Pickup Authorization
I understand that only the parent/guardian or individuals listed on my approved pickup list may pick up my child.
8. Agreement
By signing below, I confirm that I have read, understood, and agree to all terms of this waiver and consent.
